Help! I think my Blue tang LOST its EYESIGHT!!

I have a blue tang that is with me for more than 10 months.

A few days ago i realised that it always hid itself in this hole even why the lights are on. Its colour is very very pale, look as though its sleeping at night. When it finally came out of the hole, it bump itself into everything that comes its way.. LR, filter, walls, etc..

Try feeding but there is no improvement. There is no white spots or tattered fins. There is another yellow and purple tang but they look great.

i have mushrooms, buttons, a clam, sun coral and 2 anemones..

WHAT do you guys think happened?? Possible to be any sting from the softies? or disease?

any physical damage on the eye's itself ??

No physical damage. From what i can see, there's nothing wrong on the outside.. its still clear, not cloudy..
