Thanks for spotting my mistake, I've been over simplifying things until I got confused myself.

The other OH- I've left out reacts with the H+ in the last reaction to give H2O.

Just in case you are still confused or I'm not clear, I've attached a scan of the reactions below. As you can see, to get from Carbon dioxide to carbonate, 2 hydrogen ions are generated and these need to be neutralised by the 2 hydroxyl (OH-) groups from the calcium hydroxide.
