Hammer dying!

Most people will attribute corals dying to poor water quality and completely forget about other factors that could cause its death. Predation, protozoan attacks, water flow rate and lighting could all lead to death for the coral.

Other than just checking water parameters, I suggest you also check the following:

1. Any of your fishes taking nibs at your hammer? Injured tissue could lead to protozoan attack.

2. Is the water flow around the coral too strong. Hammer doesn't necessarily appreciate strong water flow.

3. Has the coral getting enough light or has it been shaded?

If you suspect protozoan infection, you can dip the coral in freshwater for a minute. That would kill all the bacteria. Then put it back into saltwater and use strong water current to flush out all the mucus produced.
