No more fish for all least a month

I have go through the cycle. All NH4, NO2 was zero and NO3 was .5 before I add in the fish. First week I add a yellow and a blue tang. 4 days later the yellow tang rip. No white spot or dot .All parameter is ok. 3 days later I add 3 lori, 2 dispar, 3 cadinal and 1 frame. Monitor for a week no problem.All eating.

Yes and no. All other fishes die when I get home. Only the blue tang I saw dashing above the water surface a few times
